Senior Case Manager - follow-up and outcomes (Cleveland, OH)

We are growing our team at Silverline Pike Community Partners and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 179 people based in Cleveland. What you will do • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Cleveland, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
