Senior Case Manager - follow-up and outcomes (Cleveland, OH)

We are growing our team at Bluepeak Vale Community Partners Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We are small enough to know everyone's name. What you will do • Help with applications and appointments for benefits enrolment. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, on a regular day shift. • On site in Cleveland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
