Senior Case Manager - follow-up and outcomes (Cleveland, OH)

Cedar Ember Early Learning runs childcare centres in Cleveland caring for infants, toddlers and preschoolers, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Cleveland. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for family communication. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Cleveland. • We reply to every applicant.
