Senior Case Manager - follow-up and outcomes (Cleveland, OH)

Highbridge Basin Health provides primary and specialty care in Cleveland. We are hiring a Senior Case Manager to take on follow-up and outcomes. Clinicians and administrators work side by side. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Full-time, Monday to Friday. • On site in Cleveland. • Applications are reviewed on a rolling basis.
