Senior Case Manager - follow-up and outcomes (Chicago, IL)

We are growing our team at Bluepeak Thistle Early Learning and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 518 people based in Chicago. What you will do • Help with applications and appointments for classroom supplies. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Chicago. • We will tell you what to expect at each step of the hiring process.
