Senior Case Manager - follow-up and outcomes (Chicago, IL)

Greenridge River Property Management looks after rental homes across the Chicago area. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 97 people based in Chicago. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for unit turnovers. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Chicago, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
