Senior Case Manager - follow-up and outcomes (Charlotte, NC)

Northwind River Learning Group is an education provider in Charlotte serving students and families, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 318 people based in Charlotte. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for student accounts. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with student accounts. • Bilingual English and Spanish.
