Senior Case Manager - follow-up and outcomes (Charleston, SC)

Greenridge Bright Community Partners Group runs community programmes across Charleston. We are hiring a Senior Case Manager to take on follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Charleston. • We will tell you what to expect at each step of the hiring process.
