Senior Case Manager - follow-up and outcomes (Charleston, SC)

We are growing our team at Silverline Amber Early Learning and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We celebrate small wins and steady routines. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish.
