Senior Case Manager - follow-up and outcomes (Boise, ID)

We are growing our team at Lumen Ashton Civic Services Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Benefits and stability attract long tenure. What you will do • Help with applications and appointments for constituent requests. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Boise, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
