Senior Case Manager - follow-up and outcomes (Boise, ID)

Highbridge Cedar Civic Services Partners works with county and city offices around Boise, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for constituent requests. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Boise, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
