Senior Case Manager - follow-up and outcomes (Birmingham, AL)

We are growing our team at Stonegate Vale Housing and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for lease compliance. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Birmingham, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
