Senior Case Manager - follow-up and outcomes (Austin, TX)

Cedar Union Care Services is a social-care provider serving older adults and people with disabilities around Austin. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 41 people based in Austin. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Austin, with some flexibility after the first few months. • We reply to every applicant.
