Senior Case Manager - follow-up and outcomes (Austin, TX)

Amberfield Thistle Housing provides affordable homes and support services around Austin, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for waiting lists. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Austin. • We reply to every applicant.
