Senior Case Manager - follow-up and outcomes (Austin, TX)

We are growing our team at Harborlight Fox Learning Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Families are partners. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for attendance records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Austin. • Applications are reviewed on a rolling basis.
