Senior Case Manager - follow-up and outcomes (Anchorage, AK)

We are growing our team at Nova Cross Health Partners and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Help with applications and appointments for patient education. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Anchorage. • Applications are reviewed on a rolling basis.
