Senior Case Manager - follow-up and outcomes (Albuquerque, NM)

Ironwood Cross Health is a healthcare provider in Albuquerque with clinics and support teams, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 747 people based in Albuquerque. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for insurance authorisations. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Albuquerque. • We will tell you what to expect at each step of the hiring process.
