Senior Case Manager - care planning (Wichita, KS)

Clearpath Rowan Health runs clinics and patient services around Wichita. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Wichita. • Applications are reviewed on a rolling basis.
