Senior Case Manager - care planning (Toledo, OH)

Stonegate Bridge Civic Services provides shared administrative services to local governments around Toledo, and we need a Senior Case Manager to lead our care planning. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for permits. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Toledo. • Applications are reviewed on a rolling basis.
