Senior Case Manager - care planning (Tampa, FL)

Fernwood Field Learning Group is looking for a Senior Case Manager. The job centres on care planning for school calendars, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish.
