Senior Case Manager - care planning (St. Louis, MO)

Ironwood Loom Early Learning is an early-learning provider in St. Louis, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 181 people based in St. Louis. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for meal programmes. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with meal programmes. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in St. Louis, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
