Senior Case Manager - care planning (St. Louis, MO)

Greenridge Sparrow Care Services is looking for a Senior Case Manager. The job centres on care planning for caregiver training, and it would suit someone who likes steady, practical work. You would join a small organisation of about 41 people based in St. Louis. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in St. Louis, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
