Senior Case Manager - care planning (Seattle, WA)

Clearpath Thistle Learning Group is looking for a Senior Case Manager. The job centres on care planning for school calendars, and it would suit someone who likes steady, practical work. We follow the school calendar and we like people who like learning. What you will do • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Seattle. • We will tell you what to expect at each step of the hiring process.
