Senior Case Manager - care planning (Seattle, WA)

Clearpath Thistle Learning Group is looking for a Senior Case Manager. The job centres on care planning for enrolment, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for enrolment. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, with a steady weekday schedule. • On site in Seattle. • Applications are reviewed on a rolling basis.
