Senior Case Manager - care planning (Seattle, WA)

Bluepeak Meadow Community Partners is looking for a Senior Case Manager. The job centres on care planning for grant reporting, and it would suit someone who likes steady, practical work. You would join a small organisation of about 38 people based in Seattle. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Seattle. • Applications are reviewed on a rolling basis.
