Senior Case Manager - care planning (San Jose, CA)

Rivermark Summit Property Management is looking for a Senior Case Manager. The job centres on care planning for unit turnovers, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for unit turnovers. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in San Jose. • Applications are reviewed on a rolling basis.
