Senior Case Manager - care planning (San Jose, CA)

Silverline Cross Health provides primary and specialty care in San Jose, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 18 people based in San Jose. What you will do • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies.
