Senior Case Manager - care planning (San Diego, CA)

Bluepeak Haven Learning Group runs classrooms and tutoring across the San Diego area, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of about 736 people based in San Diego. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for school calendars. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in San Diego. • We will tell you what to expect at each step of the hiring process.
