Senior Case Manager - care planning (San Antonio, TX)

Bluepeak Rowan Health Group provides primary and specialty care in San Antonio, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in San Antonio, with some flexibility after the first few months. • We reply to every applicant.
