Senior Case Manager - care planning (San Antonio, TX)

Northwind Granite Housing manages affordable housing communities across San Antonio. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 63 people based in San Antonio. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in San Antonio. • We reply to every applicant.
