Senior Case Manager - care planning (Salt Lake City, UT)

Bluepeak Corner Learning Group is looking for a Senior Case Manager. The job centres on care planning for learning materials, and it would suit someone who likes steady, practical work. Families are partners. What you will do • Help with applications and appointments for learning materials. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Salt Lake City, with some flexibility after the first few months. • We reply to every applicant.
