Senior Case Manager - care planning (Salt Lake City, UT)

Lumen Forge Health provides primary and specialty care in Salt Lake City, and we need a Senior Case Manager to lead our care planning. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Salt Lake City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
