Senior Case Manager - care planning (Rochester, NY)

Fernwood Summit Property Management manages apartment communities around Rochester. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 46 people based in Rochester. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident events. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with resident events. • Bilingual English and Spanish.
