Senior Case Manager - care planning (Rochester, NY)

Redstone Quill Health is a healthcare provider in Rochester with clinics and support teams, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of about 837 people based in Rochester. What you will do • Help with applications and appointments for referrals. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Rochester. • Applications are reviewed on a rolling basis.
