Senior Case Manager - care planning (Rochester, NY)

Summit Yarrow Health Group provides primary and specialty care in Rochester, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 56 people based in Rochester. What you will do • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Rochester. • We reply to every applicant.
