Senior Case Manager - care planning (Rochester, NY)

Summit Ivory Health provides primary and specialty care in Rochester. We are hiring a Senior Case Manager to take on care planning. You would join a large organisation of more than 1,000 people based in Rochester. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for referrals. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish.
