Senior Case Manager - care planning (Rochester, NY)

Summit Pine Community Partners is looking for a Senior Case Manager. The job centres on care planning for grant reporting, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Rochester, with some flexibility after the first few months. • We reply to every applicant.
