Senior Case Manager - care planning (Rochester, NY)

Ironwood Beacon Community Partners is a nonprofit in Rochester supporting families with food, housing and job help, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Rochester. • We reply to every applicant.
