Bright Cross Health is looking for a Senior Case Manager. The job centres on care planning for referrals, and it would suit someone who likes steady, practical work. You would join a large organisation of about 740 people based in Rochester. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Full-time, with a steady weekday schedule. • On site in Rochester, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
