Senior Case Manager - care planning (remote)

Ironwood Fern Care Services supports families and individuals in Portland with personal care and coordination. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 40 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Portland. • We will tell you what to expect at each step of the hiring process.
