Senior Case Manager - care planning (remote)

Ironwood Linden Care Services provides in-home and community support in Memphis, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of about 558 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Memphis. • We will tell you what to expect at each step of the hiring process.
