Senior Case Manager - care planning (remote)

Foxglove Lakeside Civic Services supports public agencies in Fort Worth with budgeting, records and programmes, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 26 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for permits. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with an occasional day on site in Fort Worth. • We will tell you what to expect at each step of the hiring process.
