Senior Case Manager - care planning (remote)

Summit Ivory Health Group is looking for a Senior Case Manager. The job centres on care planning for patient scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling.
