Senior Case Manager - care planning (remote)

Bright Maple Housing Group is looking for a Senior Case Manager. The job centres on care planning for maintenance stores, and it would suit someone who likes steady, practical work. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for maintenance stores. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish.
