Senior Case Manager - care planning (remote)

Bright Drift Civic Services Group works with county and city offices around Jacksonville, and we need a Senior Case Manager to lead our care planning. Public service means patience, paperwork and pride in the outcome. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for budget submissions. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Jacksonville. • We will tell you what to expect at each step of the hiring process.
