Senior Case Manager - care planning (remote)

We are growing our team at Redstone Haven Learning Group and adding a Senior Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for attendance records. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with a monthly team day in Omaha. • Applications are reviewed on a rolling basis.
