Senior Case Manager - care planning (remote)

Summit Lantern Learning Group is looking for a Senior Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 26 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Philadelphia. • Applications are reviewed on a rolling basis.
