Senior Case Manager - care planning (remote)

Silverline Bright Community Partners is a Milwaukee charity working with residents on basic needs and opportunity, and we need a Senior Case Manager to lead our care planning. Mission matters here, and so does looking after staff. What you will do • Help with applications and appointments for benefits enrolment. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for three months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Milwaukee. • We reply to every applicant.
