Senior Case Manager - care planning (remote)

Nova Lantern Learning Group is an education provider in New Orleans serving students and families, and we need a Senior Case Manager to lead our care planning. We follow the school calendar and we like people who like learning. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for attendance records. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with an occasional day on site in New Orleans. • We reply to every applicant.
