Senior Case Manager - care planning (remote)

Harborlight Pine Community Partners is a nonprofit in Birmingham supporting families with food, housing and job help, and we need a Senior Case Manager to lead our care planning. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Remote-friendly, with quarterly visits in Birmingham. • We will tell you what to expect at each step of the hiring process.
